PASSPORT INFORMATION

You can fill out the form online at the links listed below and take them to the passport office.

• You will need an official birth certificate
• You will need 2 passport photos (these can be taken while you wait at - Walgreen's - $8 / Harold's $11 / Passport Office $15)

The Passport office is located at the downtown Sioux City Post Office - It closes at 2:45 each day
When you fill out the form - we are looking at leaving on March 18, 2007 for 2 weeks to PNG

• Cost for an original Passport is $97 (Passport Processing - $55, Application - $30, Security Fee $12)
• Expedited processing is $60 extra - but we do not advise that at this time
• You will need to make out 2 separate checks to pay for the Application
